Hi there!
I just started medical billing for an oral surgeon. Where do I indicate anesthesia minutes, on the medical claim form, for cpt code 00170 cdt D9222, & D9223

Great question. Typically this is reported by 1 unit per 15 minute increment in field 24g on the line items for the anesthesia.
You can also enter the total (exact) minutes in field 19. If you wish.
