D4273 - autogenous connective tissue graft procedure (including donor and recipient surgical sites) first tooth, implant, or edentulous tooth position in graft
D4283 - autogenous connective tissue graft procedure (including donor and recipient surgical sites) – each additional contiguous tooth, implant or edentulous tooth position in same graft site
can be crosscoded to:
41870 - periodontal mucosal graft
41899 - Unlisted procedure, dentoalveolar structures
* be sure to include a narrative report describing the procedure
