Can you please let me know the medical codes for insurance predetermination for the following dental codes:
D6056
D6058
D6245
I need CPT code or HCPCS code. Do I need a modifier?

D6056 - prefabricated abutment - includes modification and placement
D6058 - abutment supported porcelain/ceramic crown
D6245 - pontic - porcelain/ceramic
There are not direct crosscodes for crowns and abutments, so you can either bill the "D" code on the medical claim (many insurers these days will process "D" codes when they are medically necessary services), or you can use the CPT code below and include a narrative report describing the procedure:
41899 - Unlisted procedure, dentoalveolar structures
There are no standard modifiers to use for this billing scenario.
