Hi!
We are looking to obtain Prior-Auth and then bill the final stage of a Full Mouth Reconstruction to CareFirst. I want to include 99070 in the Prior-Auth request but I am lost on looking up HCPCS II codes for resorbable and non resorbable collagen membrane, Abutments, titanium mesh, and vertical titanium screws to stabilize the jaw. Also, how/where would I report it on the 1500 form? And help with units?
