Should we be using the individual or group NPI on claim forms for TMJ splints and/or sleep appliances?

Hi Molar 33!
You will usually use both!
Generally, the group NPI will be used for the service facility NPI (Box 32), while the individual NPI will be used for the rendering & billing provider" (Box 24J + 33).
