Hi,
In Health insurance claim form 1500(02-12)section 11(d) do we need to say yes if the patient has dental insurance or it just applies to another health insurance coverage only, needs some clearance? thank you.

Great question. That just applies to other medical insurance, no need to list dental insurance plans there!
