My top front tooth has been shortening over time and now has pain. I was told that the root disintegrated and the tooth has fused to the bone. I need to have implant to correct the problem and relieve pain. What medical codes should be used to ensure medical coverage?

The codes themselves will not ensure medical coverage - you will want to check with your medical insurer in advance to confirm coverage for these services. If you could reply with the list of codes/services planned by your dentist, I am happy to offer some crosscoding options.
