Hi,

How can I submit for a pre-determination if the patient needs multi-phase procedures? In other words, if the next procedures need to be done weeks or months after the previous ones. It is difficult for the patient to go ahead on a treatment if he/she doesn't know if the subsequent procedures/phases will be covered. Some dental procedures don't have medical codes other than 41899. How can I use this code on the same or separate 1500 form without the ins denying it because of duplicate service?
