It is always best to check the insurer's medical policy relating to implant surgery to verify coverage details of the policy. You can normally view these on the insurer's website by visiting the provider's page and locating the "medical policies", "coverage guidelines", or "policies, procedures, & guidelines". Search the surgical policies relating to implant surgery or the CPT or ICD-9 code you are using!

Just the surgery is generally covered by medical and is referred to as implant reconstruction in CPT (unless it's an accident case). Implant reconstruction should follow the guidelines of a surgical package including the surgery tp prepare for the implants, the implants and 90 days of followup care.
