We have a patient who has medical insurance that covers bone grafting "upon review". They told me to send any and all information for the medical necessity for the bone grafting. Can you/someone get me started on a narrative for the medical necessity of a bone graft for future implant placement for sending the procedure to medical insurance? Thanks!!

Hi Guest,
You will want to generate a SOAP narrative report showing the Medical Necessity.
S- Subjective (Pt. information- chief complaints, medical history, medications, etc)
O- Objective (Drs. finding from the exam)
A- Assessment (ICD diagnosis)
P- Plan (what the treatment plan consists of)
